The photo is a bit blurry, but the lady was bouncing the baby on her back. Even in big cities, carrying children on your back or stomach is not surprising (in my opinion, it's a rural custom). Interestingly, they don't use carriers much (sometimes), most grandmothers just wrap the baby in a scarf and hop on their back.
